A stoppage-time own goal ensuring the hosts claimed their first point of the group stage. Both clubs are operating under caretaker coaches, with Chiefs guided by Khalil Ben Youssef and Cedric Kaze, while Zamalek are led by Ahmed Abdel-Raouf.
Zamalek started the match with real purpose and were rewarded inside three minutes. Seifeddine Jaziri reacted sharply to a loose ball following a set-piece and drove a low right-footed finish into the bottom corner to give the visitors an early lead.
Chiefs nearly produced an instant response when Gaston Sirino struck the right post from over 40 yards, but the playmaker was soon forced off through injury as the home side's problems grew.
Injuries to Bradley Cross and Sirino disrupted Chiefs' rhythm, yet the introduction of Mfundo Vilakazi lifted their attacking tempo.
Vilakazi drove several efforts from distance, one forcing Mohamed Sobhi into a strong low save just before half-time, but Chiefs still trailed at the break.
The second half followed a similar pattern, with Chiefs pushing forward and Zamalek threatening on the counter. Oday Dabbagh replaced the injured Jaziri and later fired narrowly wide, while Juan Alvina also tested Bruce Bvuma with a firm effort from close range.
Chiefs' best opportunities came late, with substitute Flavio Silva seeing a header saved before the match turned deep into stoppage time.
In the 90+6th minute, Nkosingiphile Ngcobo's delivery caused chaos in the Zamalek box, and goalkeeper Mohamed Sobhi diverted the ball into his own net under pressure, sending the home crowd into celebration and sealing a deserved equaliser for Chiefs.
The draw leaves Zamalek on four points from two matches, sitting second in Group D behind Al Masry, who have six. Chiefs move to one point, placing them third ahead of ZESCO United, who remain without a point.
A late twist, but a crucial one, Chiefs keep their CAF Confederation Cup campaign alive.
