With the game headed for penalties after a goalless draw at Cape Town Stadium, Chiefs had lined up Bruce Bvuma to replace Ntwari in the dying minutes of extra time.
But the 25-year-old stood his ground and refused to come off, forcing the technical team to abort the change.
Chiefs went on to lose the shootout 5-4 and Kaze insisted that this matter would be resolved within the club structures.
"I believe in those kind of situations it's something that we're gonna handle internally but he felt that he was comfortable to go on the penalties and help the team. It was coming from a good intention, maybe the way wasn't as good as it should be but it's something that we'll address internally," Kaze said.
The loss compounds a difficult run for Amakhosi, who are now winless in four domestic matches.
The coaching setup itself is also in flux, with head coach Nasreddine Nabi — the man who brought Kaze and fellow assistant Khalil Ben Youssef to Naturena — seemingly no longer in the country and his future looking uncertain.
Asked about their own positions, Kaze says that’s a matter for the club’s management.
"I believe that's a question that should be directed to the management and from this time, we are the coaches of Kaizer Chiefs and until further notice or something else decided by the management. I'm pretty sure that they will let everyone know," the coach added.
