The Lions have endured a difficult overseas tour following the death of lock Hyron Andrews earlier this month, with Saturday's clash marking their final assignment before returning home.
Van den Berg the only new face in the starting backline - albeit a very welcome one - taking over from Nico Steyn, who drops to the replacements bench to provide cover.
SJ Kotze will start at loosehead prop, joining Tiaan Lange at hooker and RF Schoeman at tighthead in an entirely revamped front row for the fifth-round encounter.
There is also a return in the second row, where Etienne Oosthuizen is back in the starting XV after being forced off during last week's match against Edinburgh.
Francke Horn retains his place in the loose trio and will continue to lead the side, with Siba Qoma returning from concussion alongside the athletic Siba Mahashe to complete the loose-forward combination.
Van Rooyen has also included several experienced options among his replacements, with Boan Venter, Sikhumbuzo Notshe and Boeta Chamberlain all set to offer additional impact in the second half.
