The 29-year-old defender, who has spent nearly a decade at Leverkusen and recently helped the club clinch a historic Bundesliga title under Xabi Alonso, is set to join the Bavarian giants in a bid to bolster their struggling backline.
Bayern Munich's Supervisory Board has already approved the deal, and sporting director Max Eberl has been given the green light to finalise the transfer.
While the deal has not been officially announced, it is understood to be imminent, with all signs pointing towards Tah's arrival in Munich.
Crucially, no transfer fee will be involved due to a clause in Tah's current contract that permits him to leave on a free.
The experienced centre-back will, however, receive a signing bonus and is expected to sign a contract running until 2028.
Tah's departure from Leverkusen was marked during a 4-2 defeat to Borussia Dortmund at the BayArena, where he gave a heartfelt farewell speech and was honoured with an official send-off.
The moment capped off a distinguished spell at the club, where he became a cornerstone of the squad.
