Alajbegovic, who can operate as both a winger and an attacking midfielder, has earned his move back to the BayArena following an impressive period with RB Salzburg. His performances there convinced Leverkusen to trigger a buy-back clause and secure his services once again.
The German side had only sold the player last summer in a deal reported to be worth four million euros, but they have now paid double that figure - around eight million - to bring him back.
The existence of the clause allowed Leverkusen to retain control over his long-term development, a decision that now appears justified.
"Kerim has not only met the high expectations set for him in Salzburg - he has exceeded them in record time," said Bayer 04 sporting managing director Simon Rolfes.
"Despite his young age, he has become a key player for RB Salzburg and proven himself on the international stage. We are confident he will play a vital role for us in the coming years."
Upon his return, Alajbegovic will sign a long-term contract running until 2031, underlining the club's belief in his potential.
"For me, it's a leap to a new level. I know the club and its high standards," said Alajbegovic.
"Bayer 04 Leverkusen is a top-16 club in Europe that consistently competes at the top of the Bundesliga. Many young players have made their mark here, and I want to do the same."
